Physical Therapist (PT) – Home Health

Position Details

  • Schedule: Monday – Friday, 8:00 AM – 5:00 PM
  • Productivity Requirement: 31.5 points per week (40 hours)
  • Mileage Reimbursement: $0.41 per mile
  • Coverage Area: Rockingham and Caswell Counties, NC
  • License Requirement: Active North Carolina Physical Therapist License or Compact License

Required Qualifications

  • Home Health experience required
  • HCHB (Homecare Homebase) experience required
  • OASIS documentation experience required
  • Strong clinical assessment and care planning skills
  • Excellent communication and patient education abilities

Position Summary


The Physical Therapist (PT) is responsible for evaluating and treating patients in the home health setting according to physician orders and established care plans. The PT develops individualized treatment plans designed to maximize patient independence, mobility, and overall functional outcomes while ensuring compliance with company policies and regulatory requirements.

Key Responsibilities

  • Perform comprehensive physical therapy evaluations and assessments.
  • Review medical records, diagnoses, precautions, and contraindications before initiating treatment.
  • Develop measurable goals and individualized treatment plans based on patient needs.
  • Provide skilled physical therapy interventions to improve mobility, strength, balance, and functional independence.
  • Regularly assess patient progress and modify treatment plans as needed.
  • Collaborate with physicians, interdisciplinary team members, patients, and caregivers regarding treatment goals and progress.
  • Recommend and assist with obtaining adaptive equipment to enhance patient function and safety.
  • Complete all clinical, billing, and treatment documentation accurately and within required timelines.
  • Maintain compliance with company policies, HIPAA regulations, state and federal guidelines, and professional practice standards.
  • Supervise Physical Therapist Assistants (PTAs) and therapy aides in accordance with state practice regulations.
  • Participate in staff meetings, continuing education, and professional development activities to maintain clinical excellence.
